Case ManagementStrategic Objective: Complete an assessment of support needs, and implement a plan for meeting those needs for each client enrolled in case-management only, at least one time a year.
Adult Abuse InvestigationStrategic Objective: Identify and respond to all complaints of abuse and neglect within 24 hours. Complete all investigation reports within the State-mandated, 45-day, time line.
K Plan Services - Community First Choice Option-Personal Attendant Care: includes bathing, dressing, eating, mobility, bowel & bladder care, stand-by support and behavior supports, medication management, budgeting, shopping, meal prep, laundry and light housekeeping for the child or adult. For adults, it can also provide non-medical transportation and chore services.
-Relief Care for caregivers which is also called respite care
-Skills Training to help people learn to be more independent with daily activities & health related tasks.

Residential Group Homes and Foster HomesGroup Homes and Foster Homes are available for both children and adults. For children, the emphasis is on a family like Foster Home setting vs a Group Home. For adults, we offer access to either Group Homes or Foster Homes depending on the individual's needs. Group Homes are run by agencies and staffed by Direct Service Providers (DSPs) in three 8 hour shifts for 24/7 care. These include Living Opportunities , Pathway Enterprises Inc., Ashland Supportive Housing, Alternative Services of Oregon, Inc.(ASI), Southern Oregon Aspire, Partnerships in Community Living (PCL).Foster Homes can have rotating staff as well, but many have live in providers who provide round the clock supports.
Family SupportFamily Support is intended to decrease the likelihood of out-of-home placement by providing families of minor children individualized supports such as specialized equipment, consultation, training, and respite. This is a low need and hence low funded service vs the more intensive K Plan services available to eligible children.
EmploymentMany local agencies provide the opportunity for adults to be competitively employed in community settings with supports as necessary.

Support Services brokerageOur local Brokerage is Creative Supports. Brokerage services provide eligible adults living in the community supports with various service possibilities based on a person-centered plan done by a Personal Agent (PA). Services might include in-home supports, respite, community inclusion, supported living, skills training and employment.
Contracted providersJackson County partners with several different agencies to provide 24-hour residential services, employment services, and supported living. These include Living Opportunities , Pathway Enterprises Inc., Ashland Supportive Housing, Alternative Services of Oregon, Inc.(ASI), Southern Oregon Aspire, Partnerships in Community Living (PCL), DreamzWork Supported Living Services (Supported Living services only), Southern Oregon Goodwill (employment only) & Creating Community Impact (Employment, In-Home & Day Supports). Other partners include: FACT, Families for Community, Swindells Center, The Arc Jackson County, as well as 60 licensed adult and child foster providers.
